A head-to-toe assessment was immediately performed, revealing that he was mentally unconscious, raising suspicions of a possible fracture on his left arm. Meanwhile, his vital signs were assessed and documented as follows:

Furthermore, the doctor on duty, the nurse supervisor, and the next of kin were immediately informed about the patient’s current condition. The treatment orders included administering intravenous antibiotics and painkillers, cleaning wound areas with disinfectant, and closely monitoring his vital signs. An X-ray of the left arm was ordered for further medical clarification.
Mr. Mathews is presently mentally conscious, oriented to time, place, and person, and accompanied by his wife. In addition, the client will continue to be closely monitored for any further complications and to prevent any other incident. Keep the doctor updated if the patient’s condition changes. The report has been endorsed by the head nurse and handed over to the next nurse on duty.
